If you are NOT covered by any of the following plans:

You can call L.A. Care @ 866.529.2141, and they will redirect you to the accurate provider or you can call
your member services.

You will need the following:

  • Member ID, D.O.B., and address
  • Call at least 2-days before to reserve a ride
  • Book for each member of the family riding
  • The parent will need to provide a pick-up time when booking an appointment or will need to call after the appointment which can take up to 90 minutes for pick-up
  • If transportation is needed for ongoing treatment, the provider can call to have a standing order placed for 3 months
  • The provider will need to provide Member ID and D.O.B. for parent and child
  • The provider will have to provide their contact information to verify that client is coming to treatment every month
